WHAT IS MEDICAL POWER OF ATTORNEY?

A Legal Form that allows for an individual ("Agent") to act in the BEST INTEREST for another person ("Principal") for all HEALTH CARE decision-making legal under the laws of your State. In recent years, partly due to the increased amount of baby-boomers reaching the age of seniority, this form has been a useful tool to allow families to have a sole decision-maker to take care of a relative if they become in a state of unconsciousness or not able to make decisions on their own. Even though this form can be assigned to anyone, it is recommended the TRUSTED person the Principal chooses to represent them is a close relative.

 

 

The Agent is not allowed to compensate themselves for their actions. Although, the Agent may reimburse themselves for all travel, food, and lodging expenses during the course of serving their duty to the Principal.

*Also, If you are also interested in having someone take-care of your financial decision-making or taking care of someone else's, then the Durable Power of Attorney Form is recommended.

 

REQUIREMENTS FOR LEGALITY

  • Signed and Dated by the Principal and the Agent
  • Witnessed by either Two-Non Blood Related Relatives or a Notary (Highly Recommended to use a Notary)
  • Parties involved must sign out of their own free will with the full intentions of granting and acting as the duties and roles required.
